{"id":308,"date":"2019-09-02T13:59:42","date_gmt":"2019-09-02T01:59:42","guid":{"rendered":"https:\/\/www.awanuilabs.co.nz\/central\/hawkesbay\/?page_id=308"},"modified":"2023-09-18T14:15:18","modified_gmt":"2023-09-18T02:15:18","slug":"home-visit-service","status":"publish","type":"page","link":"https:\/\/www.awanuilabs.co.nz\/central\/hawkesbay\/referrers\/home-visit-service\/","title":{"rendered":"Home Visit Services"},"content":{"rendered":"\n<p>To request a home visit please complete a Lab Request Form and email to: <a href=\"mailto:hbayhomevisit@awanuilabs.co.nz\">hbayhomevisit@awanuilabs.co.nz<\/a><\/p>\n\n\n\n<p><em>Please record clearly on the form that a home visit is required and the day needed if the timing is important.<\/em><\/p>\n\n\n\n<div style=\"color:#ddd\" class=\"wp-block-atomic-blocks-ab-spacer ab-block-spacer ab-divider-solid ab-divider-size-1\"><hr style=\"height:30px\"\/><\/div>\n\n\n\n<div class=\"wp-block-atomic-blocks-ab-accordion ab-block-accordion\"><details><summary class=\"ab-accordion-title\">Home Visit Eligibility Criteria<\/summary><div class=\"ab-accordion-text\">\n<p>To be eligible for a home visit, patients must:<\/p>\n\n\n\n<ul>\n<li>Be housebound due to ill health or have impaired mobility; be unable to manage everyday tasks such as grocery and shopping errands; and have no support person to provide transport to a collection centre to be eligible for a home visit.<\/li>\n\n\n\n<li>A request for a home visit must be made by the referring medical practitioner or nurse practitioner \u2013&nbsp;<strong>not<\/strong>&nbsp;the patient, their caregiver or relative.<\/li>\n\n\n\n<li>Any changes to a home visit request must be communicated to us by the referring medical practitioner or nurse practitioner \u2013&nbsp;<strong>not<\/strong>&nbsp;the patient \/ caregiver or relative<\/li>\n\n\n\n<li>Bookings are restricted to patients living within a 20km radius of the nearest Awanui Labs collection centre.<\/li>\n\n\n\n<li>Bookings will not be accepted for patients requiring restraint&nbsp;<em>unless<\/em>&nbsp;assistance is available.<\/li>\n<\/ul>\n<\/div><\/details><\/div>\n\n\n\n<div class=\"wp-block-atomic-blocks-ab-accordion ab-block-accordion\"><details><summary class=\"ab-accordion-title\">Request a Home Visit<\/summary><div class=\"ab-accordion-text\">\n<ul>\n<li>Requests for a home visit must be made by the referring medical practitioner or nurse practitioner \u2013&nbsp;<strong>not<\/strong>&nbsp;the patient, caregiver or relative.<\/li>\n\n\n\n<li>The home visit request form must not be given to the patient to present.<\/li>\n\n\n\n<li>Please make requests for a home visit at least 24 hours in advance of the requested booking day (Tuesday to Friday). Requests for a Monday&nbsp;<strong>must<\/strong>&nbsp;be made by 5pm on the preceding Thursday to allow for scheduling.<\/li>\n\n\n\n<li>We require the Home Visit Coordinator to be notified if there are any particular specifications with regard to the testing e.g. fasting or drug levels.<\/li>\n\n\n\n<li>Fill in\u00a0Lab Request Form\u00a0and email to\u00a0<a href=\"mailto:hbayhomevisit@sclabs.co.nz\"><\/a><a href=\"mailto:hbayhomevisit@awanuilabs.co.nz\">hbayhomevisit@awanuilabs.co.nz<\/a> and phone 06 872 6450<\/li>\n<\/ul>\n<\/div><\/details><\/div>\n\n\n\n<div class=\"wp-block-atomic-blocks-ab-accordion ab-block-accordion\"><details><summary class=\"ab-accordion-title\">Urgent Requests<\/summary><div class=\"ab-accordion-text\">\n<ul>\n<li>To make an Urgent Request, please phone 06 872 6450<\/li>\n\n\n\n<li>Note: These requests must be&nbsp;<em>clinically<\/em>&nbsp;urgent.<\/li>\n<\/ul>\n<\/div><\/details><\/div>\n\n\n\n<div class=\"wp-block-atomic-blocks-ab-accordion ab-block-accordion\"><details><summary class=\"ab-accordion-title\">Hours of Service<\/summary><div class=\"ab-accordion-text\">\n<ul>\n<li>Monday to Friday (excluding Public Holidays) 7.30am \u2013 4.00pm.<\/li>\n\n\n\n<li>Note: It is not possible to arrange a set time for a home visit.<\/li>\n<\/ul>\n<\/div><\/details><\/div>\n\n\n\n<div class=\"wp-block-atomic-blocks-ab-accordion ab-block-accordion\"><details><summary class=\"ab-accordion-title\">Home Visit Management<\/summary><div class=\"ab-accordion-text\">\n<ul>\n<li>If the patient is not home when we call, Awanui Labs reserves the right to request the patient to attend one of our collection rooms in the area.<\/li>\n\n\n\n<li>Patients or caregivers should inform their referring medical practitioner or nurse practitioner as soon as possible if circumstances change that could affect their eligibility for a home visit e.g. patient admitted to hospital etc.<\/li>\n\n\n\n<li>The medical practitioner or nurse practitioner must contact Awanui Labs as soon as possible should there be any changes to their patient\u2019s requirements.<\/li>\n\n\n\n<li>Patients who are considered to no longer require home visits will be discharged from the service&nbsp;<em>after consultation<\/em>&nbsp;with the referrer.<\/li>\n<\/ul>\n<\/div><\/details><\/div>\n\n\n\n<div class=\"wp-block-atomic-blocks-ab-accordion ab-block-accordion\"><details><summary class=\"ab-accordion-title\">Rest Homes<\/summary><div class=\"ab-accordion-text\">\n<ul>\n<li>We&nbsp;<strong>require<\/strong>&nbsp;a rest home staff member to be available to assist with identification as many patients are unable to self-identify.<\/li>\n\n\n\n<li>The rest home visit&nbsp;<strong>will not proceed<\/strong>&nbsp;if the Phlebotomist is not able to positively identify the patient.<\/li>\n<\/ul>\n<\/div><\/details><\/div>\n\n\n\n<div class=\"wp-block-atomic-blocks-ab-accordion ab-block-accordion\"><details><summary class=\"ab-accordion-title\">Private Homes<\/summary><div class=\"ab-accordion-text\">\n<ul>\n<li>Patients must ensure that all pets are restrained and that the mobile Phlebotomist can safely gain access to the patient\u2019s home.<\/li>\n\n\n\n<li>Patients not home at the time of a visit will be advised to contact their referring medical practitioner or nurse practitioner as soon as possible to avoid the cancellation of their home visit.
