{"id":1149,"date":"2019-03-19T04:15:39","date_gmt":"2019-03-19T04:15:39","guid":{"rendered":"https:\/\/www.awanuilabs.co.nz\/central\/wellington\/?page_id=1149"},"modified":"2025-11-04T15:33:39","modified_gmt":"2025-11-04T02:33:39","slug":"house-calls","status":"publish","type":"page","link":"https:\/\/www.awanuilabs.co.nz\/central\/wellington\/for-referrers\/specimen-collection-info\/house-calls\/","title":{"rendered":"Home Visit Service"},"content":{"rendered":"\n<p>Awanui Labs Wellington provides Home Visit collection services based on the eligibility criteria set by Health New Zealand | Te Whatu Ora for:<\/p>\n\n\n\n<ul>\n<li>Patients who are completely bedbound and unable to access one of our collection centres<\/li>\n<\/ul>\n\n\n\n<p>Patients who are able to attend GP clinics, hospital outpatients or day stay appointments without ambulance transport are not considered eligible for home visits under the current funding guidelines.&nbsp; Age, wheelchair use, or disability alone does not automatically meet the criteria. Financial hardship and\/or lack of transport are not considered acceptable criteria.<\/p>\n\n\n\n<p>Our collection centres are wheelchair accessible and often better equipped to support patients with additional needs.&nbsp; If your patient has a specific medical or behavioural barrier that may prevent safe or appropriate access to a collection centre, we welcome further details for review.<\/p>\n\n\n\n<p>If you are unsure whether a patient qualifies or would like to discuss a case, please contact our Home Visit Administrator:<\/p>\n\n\n\n<ul>\n<li>Email: <a href=\"mailto:wlg.homevisits@awanuilabs.co.nz\">wlg.homevisits@awanuilabs.co.nz<\/a><\/li>\n\n\n\n<li>Phone: 04 381 5900 (Option 3)<\/li>\n<\/ul>\n\n\n\n<div class=\"wp-block-atomic-blocks-ab-accordion ab-block-accordion\"><details><summary class=\"ab-accordion-title\"><strong>Operating hours<\/strong><\/summary><div class=\"ab-accordion-text\">\n<ul id=\"block-662da377-3440-425e-9b13-2a61edd94535\">\n<li>Monday to Friday 8.00am-4.00pm.&nbsp; The home visit service does not operate on weekends or public holidays.<\/li>\n\n\n\n<li>Serial home visits with a due date that fall on a public holiday will be rescheduled (usually to the business day prior or the next business day after the due date).<\/li>\n<\/ul>\n<\/div><\/details><\/div>\n\n\n\n<div class=\"wp-block-atomic-blocks-ab-accordion ab-block-accordion\"><details><summary class=\"ab-accordion-title\"><strong>Requesting a home visit<\/strong><\/summary><div class=\"ab-accordion-text\">\n<ul>\n<li>Patients, family members, or caregivers are not permitted to book home visits directly. All home visit requests must be made by a healthcare provider.<\/li>\n\n\n\n<li>Any changes to the request must be communicated directly to the Home Visit Administrator by the requesting doctor or nurse and <strong>NOT by the patient or family\/caregiver.<\/strong><\/li>\n\n\n\n<li>All home visit booking requests must be received no later than 3pm on the business day prior to the date that the blood test needs to be collected.<\/li>\n\n\n\n<li>Awanui Labs is unable to accept home visit bookings that are more than<strong> two weeks in advance.<\/strong><\/li>\n\n\n\n<li>INR home visit bookings for warfarin monitoring can only be made one visit at a time. Requests for recurring or multiple bookings cannot be accommodated, as testing frequency is based on results. If a new home visit request is received while an existing INR booking is already in place, the existing appointment will be cancelled and replaced with the new request.<\/li>\n\n\n\n<li>Any change in a patients\u2019 circumstances that will affect the home visit (e.g. patient admission to hospital, change of address) must be promptly communicated to the Home Visit Administrator to avoid a wasted home visit.<\/li>\n\n\n\n<li>It is not possible to arrange specific times or time frames for a home visit.&nbsp; As these patients are, by definition housebound, this should not be cause for concern.<\/li>\n\n\n\n<li>Best endeavours will be made to accommodate the individual needs of the patients; however, these cannot be guaranteed.<\/li>\n\n\n\n<li>A home visit request cannot be booked unless a request form has been received by the Home Visit Administrator.&nbsp; We cannot accept bookings via phone call or voicemail messages; requests must be submitted by using one of the following approved methods<\/li>\n<\/ul>\n\n\n\n<p><strong>Medical centre (GP\/RN) instructions<\/strong><\/p>\n\n\n\n<ul>\n<li>Generate a blood test E-order using \u00c9CLAIR &amp; ensure that the \u2018Home Visit Request\u2019 box is ticked and complete the Home Visit information requested including a preferred collection date if required.<\/li>\n<\/ul>\n\n\n\n<p><strong>Hospital or specialist (DR\/RN) instructions<\/strong><\/p>\n\n\n\n<ul>\n<li>Scan a hardcopy blood test request form and send via email to <a href=\"mailto:wlg.homevisits@awanuilabs.co.nz\">wlg.homevisits@awanuilabs.co.nz<\/a><\/li>\n\n\n\n<li>In the email message, please include:\n<ul>\n<li> full patient details (first name, last name, DOB &amp; NHI), <\/li>\n\n\n\n<li>the physical address that we are to visit the patient, <\/li>\n\n\n\n<li>any issues with gaining access to the property (e.g. dogs onsite), <\/li>\n\n\n\n<li>preferred collection date if required and <\/li>\n\n\n\n<li>explanation\/confirmation that your patient meets the criteria for receiving a home visit (see criteria above).<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li>Please <u>only<\/u> scan the form to the Home Visit Service \u2013 DO NOT supply the patient with a copy of the form.<\/li>\n<\/ul>\n\n\n\n<p><strong>Rest home (RN) instructions<\/strong><\/p>\n\n\n\n<ul>\n<li>Using the Home Visit booking requirements form (specifically ONLY for use by rest home clinical staff), please complete the relevant sections and  follow the instructions as noted. This form can also be downloaded and printed from our <a href=\"https:\/\/fl-healthscope-media.s3.amazonaws.com\/lab-sites\/uploads\/sites\/3\/2023\/09\/Awanui-labs-Wellington-Rest-home-booking.pdf\">website here.<\/a><\/li>\n\n\n\n<li>If you are a doctor or nurse in a general practise, hospital or specialist centre requesting a home visit for your patient who lives in a rest home please DO NOT use this Rest Home form and refer to the instructions above).<\/li>\n<\/ul>\n<\/div><\/details><\/div>\n\n\n\n<div class=\"wp-block-atomic-blocks-ab-accordion ab-block-accordion\"><details><summary class=\"ab-accordion-title\"><strong>Home visit management<\/strong><\/summary><div class=\"ab-accordion-text\">\n<ul id=\"block-520ddf38-94af-40a2-8ca1-2248b5db118d\">\n<li>If the patient is confirmed to have not been at home at the time the home visit phlebotomist arrives at the address, Awanui Labs reserves the right to request the patient to attend one of our collection centres.&nbsp; Details of our collection centres and their opening hours can be found on <a href=\"https:\/\/www.awanuilabs.co.nz\/central\/wellington\/collection-centres\/\">our website here.<\/a><\/li>\n\n\n\n<li>Patients who no longer meet the eligibility criteria will be discharged from the home visit service and the referrer will be notified by email.<\/li>\n\n\n\n<li>Patients who miss three consecutive home visits due to being confirmed out at the time the Awanui phlebotomist arrives at the address will be discharged from the home visit service and the referrer will be notified by email.<\/li>\n\n\n\n<li>Awanui Labs will review regular home visit patients annually to ensure eligibility.&nbsp; A list of regular home visit patients will be provided to the referring practitioner by email for review of the continued eligibility for home visits (and the frequency of testing for serial frequency home visits where required)<\/li>\n<\/ul>\n<\/div><\/details><\/div>\n\n\n\n<div class=\"wp-block-atomic-blocks-ab-accordion ab-block-accordion\"><details><summary class=\"ab-accordion-title\"><strong>The home visit<\/strong><\/summary><div class=\"ab-accordion-text\">\n<ul id=\"block-4162321e-8363-4bc4-a453-5f391d74a405\">\n<li>Patients must ensure that all pets are restrained, and that the mobile phlebotomist can safely access the patient\u2019s home.<\/li>\n\n\n\n<li>The home visit will not proceed if the phlebotomist is unable to positively identify the patient.&nbsp; Patients unable to communicate their identification must provide photo ID or have a support person present to confirm their identity.<\/li>\n\n\n\n<li>Rest homes must provide a staff member to accompany the visiting phlebotomist for assistance and identification of their residents.<\/li>\n\n\n\n<li>Patients that do not appear to be home at the time of the visit will be left a calling card advising them to present at a collection centre for their blood test or to call the requesting doctor\/nurse in extenuating circumstances that require the home visit to be rebooked.<\/li>\n<\/ul>\n<\/div><\/details><\/div>\n\n\n\n<p>We appreciate your ongoing support in helping us deliver care to those who need it most.<\/p>\n","protected":false},"excerpt":{"rendered":"Awanui Labs Wellington provides Home Visit collection services based on the eligibility criteria set by Health New Zealand | Te Whatu Ora for: Patients who are able to attend GP clinics, hospital outpatients or day stay appointments without ambulance transport are not considered eligible for home visits under the current funding guidelines.&nbsp; Age, wheelchair use, [&hellip;]","protected":false},"author":1,"featured_media":0,"parent":549,"menu_order":0,"comment_status":"closed","ping_status":"closed","template":"page-left-sidebar.php","meta":{"footnotes":""},"yoast_head":"<!-- This site is optimized with the Yoast SEO plugin v24.2 - https:\/\/yoast.com\/wordpress\/plugins\/seo\/ -->\n<title>Home Visit Service &#8211; Awanui Labs - Wellington<\/title>\n<meta name=\"description\" content=\"Awanui Labs offers a free to the patient (CCDHB funded) home visit service for house bound patients.
