{"id":4473,"date":"2021-09-15T16:44:11","date_gmt":"2021-09-15T04:44:11","guid":{"rendered":"https:\/\/www.awanuilabs.co.nz\/central\/wellington\/?page_id=4473"},"modified":"2025-05-05T14:30:40","modified_gmt":"2025-05-05T02:30:40","slug":"congenital-syphilis","status":"publish","type":"page","link":"https:\/\/www.awanuilabs.co.nz\/central\/wellington\/for-referrers\/microbiology\/how-do-i-diagnose\/congenital-syphilis\/","title":{"rendered":"Congenital syphilis"},"content":{"rendered":"\n<p><strong>Infection:<\/strong> congenital syphilis; <em>Treponema pallidum<\/em> infection; many other historical names<\/p>\n\n\n\n<div class=\"wp-block-atomic-blocks-ab-accordion ab-block-accordion\"><details><summary class=\"ab-accordion-title\">Brief description:<\/summary><div class=\"ab-accordion-text\">\n<ul class=\"wp-block-list\">\n<li>A sexually transmitted bacterial infection that can cause serious congenital infection.<\/li>\n\n\n\n<li>Most congenitally infected infants will be asymptomatic at birth, with symptoms usually appearing over the coming months if untreated.<\/li>\n\n\n\n<li>Symptomatic infants can have a wide variety of manifestations.<\/li>\n<\/ul>\n<\/div><\/details><\/div>\n\n\n\n<div class=\"wp-block-atomic-blocks-ab-accordion ab-block-accordion\"><details><summary class=\"ab-accordion-title\">Did you know?<\/summary><div class=\"ab-accordion-text\">\n<ul class=\"wp-block-list\">\n<li>Congenital infection occurs when organisms in maternal blood infect the placenta.<\/li>\n\n\n\n<li>Organisms are more likely to be in maternal blood in early syphilis (primary\/secondary), which is why the congenital infection risk is highest at these stages.<\/li>\n\n\n\n<li>With late maternal syphilis the risk is much lower, as organisms are rarely in the blood.<\/li>\n\n\n\n<li>A larger placenta is more likely to be infected than a smaller one, which is why the risk of congenital infection goes up in the later stages of pregnancy.<\/li>\n<\/ul>\n<\/div><\/details><\/div>\n\n\n\n<div class=\"wp-block-atomic-blocks-ab-accordion ab-block-accordion\"><details><summary class=\"ab-accordion-title\">Diagnostic approach:<\/summary><div class=\"ab-accordion-text\">\n<ul class=\"wp-block-list\">\n<li>Testing is recommended as part of the standard antenatal \u2018booking bloods\u2019 in NZ<\/li>\n\n\n\n<li>Higher risk women (e.g. STI diagnosed during pregnancy, new or multiple partners during pregnancy) should be rescreened at 28-32 weeks<\/li>\n\n\n\n<li>Testing is recommended in women at any gestation with symptoms of primary infection (e.g. ulcerative skin lesions) or secondary infection (e.g. skin rash and\/or flu like symptoms)<\/li>\n\n\n\n<li>Testing is recommended in women with fetal death at &gt;20 weeks or if abnormal USS findings<\/li>\n\n\n\n<li>Testing is recommended for all neonates where congenital syphilis is a possibility<\/li>\n<\/ul>\n<\/div><\/details><\/div>\n\n\n\n<div class=\"wp-block-atomic-blocks-ab-accordion ab-block-accordion\"><details><summary class=\"ab-accordion-title\">Antenatal test of choice:<\/summary><div class=\"ab-accordion-text\">\n<p>Request <strong><a href=\"https:\/\/scg.labapps.nz\/#\/WEL\/details\/27402\" target=\"_blank\" rel=\"noreferrer noopener\">Syphilis serology<\/a><\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>This is mostly to confirm\/refute maternal infection, confirmation of congenital infection is usually done in the neonatal setting<\/li>\n\n\n\n<li>Can answer two questions:<ul><li>Has mother <strong>ever<\/strong> had infection with syphilis (including if treated\/cleared)?<ul><li><strong>EIA and TPPA<\/strong><\/li><\/ul><\/li><\/ul>\n<ul class=\"wp-block-list\">\n<li>Does mother have current <strong>active<\/strong> infection?\n<ul class=\"wp-block-list\">\n<li><strong>RPR<\/strong><\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li><strong>Syphilis EIA <\/strong>detects anti-treponemal antibodies (a <strong>\u2018treponemal\u2019 <\/strong>test). These develop <strong>shortly after primary infection<\/strong> and usually last <strong>lifelong<\/strong> i.e. indicate infection <strong>at some stage<\/strong>.<ul><li>Excellent<strong> sensitivity &#8211; <\/strong>a<strong> negative <\/strong>result<strong> excludes <\/strong>syphilis unless<strong> very early in<\/strong>fection\/treatment, or rarelyin HIV\/immune compromise<\/li><\/ul>\n<ul class=\"wp-block-list\">\n<li>Good <strong>specificity<\/strong> &#8211; however, a second test is used to <strong>confirm<\/strong> a positive result, as <strong>false positives<\/strong> can occur.\n<ul class=\"wp-block-list\">\n<li><strong>TPPA<\/strong> (another <strong>\u2018treponemal\u2019<\/strong> test) is used for this purpose\n<ul class=\"wp-block-list\">\n<li>EIA<strong> pos <\/strong>&amp;TPPA<strong> pos <\/strong>= treponemal infection <strong>at some stage. <\/strong>Note,<strong> <\/strong>other treponemal infections, e.g. <strong>Yaws<\/strong>, give the <strong>same result.<\/strong><\/li>\n\n\n\n<li>EIA<strong> pos <\/strong>&amp;TPPA<strong> neg&nbsp;<\/strong>=<strong> false positive EIA<\/strong> (most common explanation) or <strong>very early infection.<\/strong><\/li>\n<\/ul>\n<\/li>\n\n\n\n<li><strong>RPR<\/strong> detects non-specific antibodies (a \u2018<strong>non-treponemal<\/strong>\u2019 test) caused by <strong>active infection<\/strong>.<ul><li>This is <strong>added on automatically<\/strong> by the lab if EIA positive (even if TPPA negative).<\/li><\/ul>\n<ul class=\"wp-block-list\">\n<li>Unlike EIA\/TPPA, which remain positive regardless, the value of this test <strong>changes over time<\/strong> due to <strong>treatment<\/strong> or the <strong>immune response<\/strong> to infection.\n<ul class=\"wp-block-list\">\n<li>Used to assess for <strong>inactive vs active infection<\/strong> and <strong>response to treatment<\/strong>.<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li>Any pregnant woman with <strong>positive EIA &amp; TPPA<\/strong> (regardless of RPR) should be <strong>discussed with a relevant specialist<\/strong> regarding the need for treatment<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<\/div><\/details><\/div>\n\n\n\n<div class=\"wp-block-atomic-blocks-ab-accordion ab-block-accordion\"><details><summary class=\"ab-accordion-title\"><strong>Postnatal diagnosis<\/strong><\/summary><div class=\"ab-accordion-text\">\n<ul class=\"wp-block-list\">\n<li>Testing is complicated and should be under <strong>specialist guidance <\/strong>and combined with <strong>maternal history <\/strong>and infant <strong>physical examination<\/strong>.<\/li>\n\n\n\n<li>It is often not possible to confidently rule in or rule out the diagnosis perinatally, so the pre-test probability (i.e. maternal treatment and exposure history) often influences treatment decisions more than testing.<\/li>\n\n\n\n<li><strong><a href=\"https:\/\/scg.labapps.nz\/#\/WEL\/details\/27402\" target=\"_blank\" rel=\"noreferrer noopener\">Syphilis serology<\/a><\/strong>:\n<ul class=\"wp-block-list\">\n<li><strong>Infant RPR<\/strong> tested in parallel to <strong>maternal RPR<\/strong><ul><li>Should not be performed on cord blood samples (risk of maternal blood interference)<\/li><\/ul>\n<ul class=\"wp-block-list\">\n<li>Infant <strong>RPR <\/strong><strong>\u22654-fold higher<\/strong> than maternal RPR considered evidence of proven\/highly probable congenital syphilis\n<ul class=\"wp-block-list\">\n<li>RPR &lt;4-fold higher is less definitive, doesn\u2019t rule in or out<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li><strong>Placental investigations<\/strong>:\n<ul class=\"wp-block-list\">\n<li><strong>Histopathology + PCR<\/strong><ul><li>Certain features can be highly specific so make congenital syphilis likely<\/li><\/ul><ul><li>Can <strong>confirm the diagnosis<\/strong> if combined with <strong>immunohistochemical stains<\/strong> or <strong>PCR<\/strong> demonstrating the organism.<\/li><\/ul>\n<ul class=\"wp-block-list\">\n<li>Sensitivity is less certain, so a <strong>normal result doesn\u2019t exclude the diagnosis<\/strong>.<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li><strong><a href=\"https:\/\/scg.labapps.nz\/#\/WEL\/details\/27872\" target=\"_blank\" rel=\"noreferrer noopener\">Syphilis PCR<\/a><\/strong><ul><li>Excellent <strong>specificity<\/strong> &#8211; a <strong>positive result confirms <\/strong>the diagnosisUncertain<strong> sensitivity &#8211; <\/strong>a<strong> negative result doesn\u2019t exclude <\/strong>the diagnosis. Can be performed on <strong>nasal discharge<\/strong>, swab of infant <strong>rash<\/strong><\/li><\/ul>\n<ul class=\"wp-block-list\">\n<li>Not yet widely available<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li><strong><a href=\"https:\/\/scg.labapps.nz\/#\/WEL\/details\/27757\" target=\"_blank\" rel=\"noreferrer noopener\">CSF testing<\/a><\/strong> (cell count, protein, VDRL\/FTA-Abs)\n<ul class=\"wp-block-list\">\n<li>Typically indicated if <strong>any abnormal findings<\/strong> on examination or above tests<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<\/div><\/details><\/div>\n\n\n\n<div class=\"wp-block-atomic-blocks-ab-accordion ab-block-accordion\"><details><summary class=\"ab-accordion-title\">Tests to avoid\/specialist tests:<\/summary><div class=\"ab-accordion-text\">\n<ul class=\"wp-block-list\">\n<li>Interval follow-up serology testing is performed, with frequency usually determined by initial results\n<ul class=\"wp-block-list\">\n<li>This should be under specialist guidance<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li>Infant syphilis IgM\n<ul class=\"wp-block-list\">\n<li>This is recommended in some guidelines, however <strong>lacks specificity<\/strong>, so in our experience rarely alters management and isn\u2019t recommended.<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<\/div><\/details><\/div>\n\n\n\n<div class=\"wp-block-atomic-blocks-ab-accordion ab-block-accordion\"><details><summary class=\"ab-accordion-title\">Other considerations:<\/summary><div class=\"ab-accordion-text\">\n<p>Note that <strong>EIA\/TPPA does not give useful information in the neonatal setting<\/strong>, as there is no way to differentiate between passively acquired maternal antibodies and the infant\u2019s own antibodies.<\/p>\n<\/div><\/details><\/div>\n","protected":false},"excerpt":{"rendered":"Infection: congenital syphilis;
