{"id":1578,"date":"2020-11-26T12:01:34","date_gmt":"2020-11-25T23:01:34","guid":{"rendered":"https:\/\/www.awanuilabs.co.nz\/north\/northland\/?page_id=1578"},"modified":"2026-01-13T10:52:40","modified_gmt":"2026-01-12T21:52:40","slug":"home-visit-service","status":"publish","type":"page","link":"https:\/\/www.awanuilabs.co.nz\/north\/northland\/for-referrers\/home-visit-service\/","title":{"rendered":"Home Visit Service"},"content":{"rendered":"\n<p>Eligibility for funded Awanui Labs Northland home visit collection services as follows:<\/p>\n\n\n\n<p>This service is only available in the Whangarei area and within our Home Visit boundaries. <\/p>\n\n\n\n<p>Patients must be bedridden or have impaired mobility, mentally unwell or physically disabled, AND have no support person to provide transport to a collection centre.<\/p>\n\n\n\n<p>If the patient is a minor, the parent or direct caregiver must be bedridden or have impaired mobility, mentally unwell or physically disabled, AND have no support person to provide transport to a collection centre.<\/p>\n\n\n\n<p>*Please note: Residents that are living independently at Rest Homes\/Villages that are mobile are not eligible for this service*<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Make a Request <\/h2>\n\n\n\n<p>To make a request for our home visit service <a href=\"https:\/\/fl-healthscope-media.s3.amazonaws.com\/lab-sites\/uploads\/sites\/4\/2025\/11\/Home-Visit-request-form-for-referrers.pdf\">download our request form.<\/a><\/p>\n\n\n\n<p>Phone: 09 438 4243  or Email:<strong> <\/strong><a href=\"mailto:nth.homevisits@awanuilabs.co.nz\">nth.homevisits@awanuilabs.co.nz<\/a><\/p>\n\n\n\n<div style=\"height:30px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<h2 class=\"wp-block-heading\">Home Visit Guidelines<\/h2>\n\n\n\n<p>Download or read our <a href=\"https:\/\/aphgconz.sharepoint.com\/:w:\/r\/sites\/LAB-Northland-PatientServices\/_layouts\/15\/Doc.aspx?sourcedoc=%7B901C6258-3DFA-471A-9580-D78705642FF1%7D&amp;file=NPL-MPM-004.docx&amp;action=default&amp;mobileredirect=true\">home visit guidelines here.<\/a><\/p>\n\n\n\n<div style=\"height:40px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<div class=\"wp-block-atomic-blocks-ab-accordion ab-block-accordion\"><details><summary class=\"ab-accordion-title\"><strong>Hours of Service<\/strong><\/summary><div class=\"ab-accordion-text\">\n<p>Monday to Friday (excluding Public Holidays) 8.00am \u2013 3.30pm.<\/p>\n\n\n\n<p>Available in Whangarei only. <\/p>\n\n\n\n<p>Note: repeat bookings that fall on a public holiday will be rescheduled and the referrer advised.<\/p>\n<\/div><\/details><\/div>\n\n\n\n<div class=\"wp-block-atomic-blocks-ab-accordion ab-block-accordion\"><details><summary class=\"ab-accordion-title\"><strong>Request a Home Visit<\/strong><\/summary><div class=\"ab-accordion-text\">\n<p>Requests for a home visit must be made by the referring practitioner or authorised representative (e.g. Senior Nurse), <strong>not<\/strong> <strong>the patient<\/strong>, using Awanui Labs home visit request form.<\/p>\n\n\n\n<p>Request forms must be emailed to Awanui Labs and <strong>not given to the patient<\/strong> as we need to sight the information on the form to complete the booking.<\/p>\n\n\n\n<p>The home visit request cannot be booked if Awanui Labs has not received the request form.<\/p>\n\n\n\n<p><strong>With the exception of urgent requests<\/strong>, 24 hours\u2019 notice is required for a home visit booking.<\/p>\n\n\n\n<p>Best endeavours will be made to accommodate the individual needs of the patients, i.e. day, and time of day, however cannot be guaranteed.<\/p>\n\n\n\n<p>Morning priority will be given to patients required to fast, or withhold medication.<\/p>\n\n\n\n<p>If an urgent home visit is required the time frame will be confirmed after consultation with the referring practitioner.<\/p>\n\n\n\n<p>If the home visit is unable to be scheduled on the requested date, the referring practitioner will be contacted.<\/p>\n\n\n\n<p>The following information must be included on request forms:<\/p>\n\n\n\n<ul>\n<li>Complete patient details: NHI, full name, date of birth and gender<\/li>\n\n\n\n<li>Complete patient address<\/li>\n\n\n\n<li>Contact telephone numbers for patient and referrer<\/li>\n\n\n\n<li>Tests required<\/li>\n\n\n\n<li>Fasting or withholding medication, and time patient normally takes medication<\/li>\n\n\n\n<li>Relevant clinical details and reason home visit is required<\/li>\n\n\n\n<li>Any additional information that would affect the home visit e.g. patient can\u2019t speak English<\/li>\n\n\n\n<li>Referrers signature<\/li>\n<\/ul>\n<\/div><\/details><\/div>\n\n\n\n<div class=\"wp-block-atomic-blocks-ab-accordion ab-block-accordion\"><details><summary class=\"ab-accordion-title\"><strong>Assessment of Patient&#8217;s Eligibility<\/strong><\/summary><div class=\"ab-accordion-text\">\n<p>Awanui Labs will review regular home visit patients annually. Eligibility for home visits is assessed to ensure we can provide home visits to those patients with the greatest need.<\/p>\n\n\n\n<p>A list of regular patients will be compiled and provided to the referring practitioner for review of the continued need for testing, the frequency of testing and eligibility for home visits.<\/p>\n<\/div><\/details><\/div>\n\n\n\n<div class=\"wp-block-atomic-blocks-ab-accordion ab-block-accordion\"><details><summary class=\"ab-accordion-title\"><strong>Discontinuation of Home Visits<\/strong><\/summary><div class=\"ab-accordion-text\">\n<p>Awanui Labs will notify the referring practitioner where there is evidence that the patient may no longer meet the home visit eligibility criteria.<\/p>\n\n\n\n<p>Awanui Labs will notify the referring practitioner if the patient is repeatedly not at home at the time of visit or where circumstances have changed enabling the patient to visit a collection centre.<\/p>\n<\/div><\/details><\/div>\n\n\n\n<div class=\"wp-block-atomic-blocks-ab-accordion ab-block-accordion\"><details><summary class=\"ab-accordion-title\"><strong>Patient Responsibility<\/strong><\/summary><div class=\"ab-accordion-text\">\n<p>Patients are not permitted to make their own home visit bookings.<\/p>\n\n\n\n<p>Patients should inform Awanui Labs as soon as possible if circumstances change that could affect their eligibility for a home visit e.g. patient admitted to hospital, moved house etc.<\/p>\n\n\n\n<p>Any changes to the request must be communicated by the referrer.<\/p>\n<\/div><\/details><\/div>\n\n\n\n<div class=\"wp-block-atomic-blocks-ab-accordion ab-block-accordion\"><details><summary class=\"ab-accordion-title\"><strong>The Home Visit<\/strong><\/summary><div class=\"ab-accordion-text\">\n<p>Patients must ensure that all pets are restrained and that the mobile phlebotomist can safely gain access to the patient\u2019s home.<\/p>\n\n\n\n<p>Patients not home at the time of a visit will be left a calling card and advised to contact their referrer as soon as possible to avoid the cancellation of their next home visit.<\/p>\n\n\n\n<p>The home visit will not proceed if the phlebotomist is not able to positively identify the patients.<\/p>\n\n\n\n<p>Patients who are unable to effectively communicate their identification, e.g. non-English speaking, must provide photo ID or have a support person present who can provide identification.<\/p>\n<\/div><\/details><\/div>\n","protected":false},"excerpt":{"rendered":"Eligibility for funded Awanui Labs Northland home visit collection services as follows: This service is only available in the Whangarei area and within our Home Visit boundaries.
