{"id":121,"date":"2018-09-13T06:19:17","date_gmt":"2018-09-13T06:19:17","guid":{"rendered":"https:\/\/www.awanuilabs.co.nz\/south\/canterbury\/?page_id=82"},"modified":"2026-08-21T10:25:28","modified_gmt":"2026-08-20T22:25:28","slug":"home-visit-service","status":"publish","type":"page","link":"https:\/\/www.awanuilabs.co.nz\/south\/canterbury\/for-referrers\/home-visit-service\/","title":{"rendered":"Home visit service"},"content":{"rendered":"\n<div class=\"wp-block-atomic-blocks-ab-accordion ab-block-accordion\"><details><summary class=\"ab-accordion-title\"><strong>Awanui Home Visit Eligibility Criteria \u2013 Information for Referrers<\/strong>\u00a0<\/summary><div class=\"ab-accordion-text\">\n<p class=\"wp-block-paragraph\">In response to increasing demand for Home Visit requests and due to several Home Visit requests unable to proceed due to patients not being at home when our phlebotomists visit, Awanui has reviewed and updated the Home Visit requesting and eligibility criteria &#8211; effective April 2026.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Please see below the updated eligibility criteria and home visit requesting requirements.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Eligibility Criteria<\/strong>&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Home visits are limited to patients who are living within a 20km radius of the nearest Awanui Labs collection centre that provides a home visit service and:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Medically housebound or bedridden, or&nbsp;<\/li>\n\n\n\n<li>Have a special medical circumstance that clearly prevents attendance at a collection centre and is supported by detailed clinical justification.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Clinical justification&nbsp;is required for Home-visit requests<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">All home visit requests must include appropriate clinical details&nbsp;explaining why the patient meets the eligibility criteria.&nbsp;Requests that do not&nbsp;contain&nbsp;sufficient clinical detail will not be scheduled.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>The Following Alone Do&nbsp;<\/strong><em><strong>Not<\/strong><\/em><strong>&nbsp;Meet Eligibility:<\/strong>&nbsp;<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Transportation difficulties&nbsp;<\/li>\n\n\n\n<li>General mobility limitations&nbsp;<\/li>\n\n\n\n<li>Advanced age without supporting medical justification&nbsp;<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">We understand these factors can create challenges for patients and wh\u0101nau; however, on their own, they do not qualify a patient for home visit services.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Ongoing Review of Eligibility<\/strong>&nbsp;<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Eligibility is subject to regular review by the Home Visit Coordinator.&nbsp;<\/li>\n\n\n\n<li>Referrers will be notified via a result comment if a patient is not eligible or no longer meets the criteria.&nbsp;<\/li>\n<\/ul>\n<\/div><\/details><\/div>\n\n\n\n<div class=\"wp-block-atomic-blocks-ab-accordion ab-block-accordion\"><details><summary class=\"ab-accordion-title\"><strong>Making a Home Visit Request<\/strong><\/summary><div class=\"ab-accordion-text\">\n<ul class=\"wp-block-list\">\n<li>The electronic e-Order form and tick box must be used for Home Visit requests. Where this is unavailable, please email <a href=\"mailto:SIHomevisits@awanuilabs.co.nz\">SIHomevisits@awanuilabs.co.nz<\/a> to discuss the process.<\/li>\n\n\n\n<li>Urgent requests <strong>MUST <\/strong>be phoned through. Please call the lab and ask for the Home Visit Coordinator. Please do not just mark a request form urgent and assume it will be classed as urgent. You must phone the lab to check availability, or it may be missed.<\/li>\n\n\n\n<li>Same-day urgent requests must be received by 12:00 pm to be considered. Requests received after this time will be scheduled for the following business or scheduled visit day.<\/li>\n\n\n\n<li>Requests are scheduled for the next available day and may be subject to change dependant on daily capacity and request urgency.<\/li>\n\n\n\n<li>Awanui does not provide a booking time and day to patients, as per the eligibility criteria the patient is expected to be at home.<\/li>\n\n\n\n<li>Morning priority will be given to patients who are fasting or withholding medication.&nbsp;<\/li>\n\n\n\n<li>Patients may be removed from the home visit service following one unsuccessful visit without proper justification.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Home Visits to Residential Aged Care Facilities<\/strong>&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">In addition to meeting the eligibility criteria above &#8211; the following also apply:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Where possible Awanui will plan routine collections at facilities once a week per facility and on the same day each week.<\/li>\n\n\n\n<li>A facility staff member must escort the phlebotomist to each patient and&nbsp;assist&nbsp;with identification and any required support.&nbsp;<\/li>\n\n\n\n<li>Patients in aged care facilities can only be referred for a home visit by their medical practitioner.&nbsp;<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Private Home Collections<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Collections will not proceed in an environment deemed unsafe by staff<\/li>\n\n\n\n<li>Pets must be restrained and safe access provided<\/li>\n\n\n\n<li>Positive patient identification is mandatory &#8211; Patients unable to self-identify must provide photo ID or have a support person present.<\/li>\n\n\n\n<li>When requesting a home visit the referring clinician must agree to the above criteria<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Missed Visits<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Referrers will be notified via a result comment if a patient is not present at the time visited<\/li>\n\n\n\n<li>The Home Visit will not automatically be rebooked &#8211; Confirmation of ongoing eligibility will be required prior to further visits<\/li>\n\n\n\n<li>Patients may be removed from the Home Visit service following one unsuccessful visit without proper justification.<\/li>\n<\/ul>\n<\/div><\/details><\/div>\n\n\n\n<div class=\"wp-block-atomic-blocks-ab-accordion ab-block-accordion\"><details><summary class=\"ab-accordion-title\">Home visits FAQs<\/summary><div class=\"ab-accordion-text\">\n<div class=\"wp-block-atomic-blocks-ab-accordion ab-block-accordion\"><details><summary class=\"ab-accordion-title\">Who qualifies for a funded home visit?<\/summary><div class=\"ab-accordion-text\">\n<p class=\"wp-block-paragraph\">Funded home visits are only approved for:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Patients who are bedridden and unable to attend a collection centre due to medical reasons, or<\/li>\n\n\n\n<li>Special medical cases, where detailed clinical justification is provided.<\/li>\n<\/ul>\n<\/div><\/details><\/div>\n\n\n\n<div class=\"wp-block-atomic-blocks-ab-accordion ab-block-accordion\"><details><summary class=\"ab-accordion-title\">Do transport difficulties or mobility issues qualify a patient for a home visit?<\/summary><div class=\"ab-accordion-text\">\n<p class=\"wp-block-paragraph\">No. Limited transport options, advanced age, or general mobility challenges do not meet the eligibility criteria. We recognise that such factors may make attending a collection centre more difficult, but unfortunately, they are not sufficient for funded home visit approval under the current Health New Zealand | Te Whatu Ora agreement.<\/p>\n<\/div><\/details><\/div>\n\n\n\n<div class=\"wp-block-atomic-blocks-ab-accordion ab-block-accordion\"><details><summary class=\"ab-accordion-title\">Are patients in wheelchairs eligible for home visits?<\/summary><div class=\"ab-accordion-text\">\n<p class=\"wp-block-paragraph\">Not by default. All Awanui collection centres are wheelchair accessible. Patients who use wheelchairs but are otherwise mobile or able to leave home with assistance are expected to attend a collection centre.<\/p>\n<\/div><\/details><\/div>\n\n\n\n<div class=\"wp-block-atomic-blocks-ab-accordion ab-block-accordion\"><details><summary class=\"ab-accordion-title\">What if the patient can attend other healthcare appointments?<\/summary><div class=\"ab-accordion-text\">\n<p class=\"wp-block-paragraph\">If a patient can attend:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Hospital outpatient appointments,<\/li>\n\n\n\n<li>Day stay units, or<\/li>\n\n\n\n<li>GP consultations in person (without ambulance transport)<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">they do not automatically qualify for a home visit.<\/p>\n<\/div><\/details><\/div>\n\n\n\n<div class=\"wp-block-atomic-blocks-ab-accordion ab-block-accordion\"><details><summary class=\"ab-accordion-title\">Do residents of retirement villages or aged residential care (ARC) facilities automatically qualify?<\/summary><div class=\"ab-accordion-text\">\n<p class=\"wp-block-paragraph\">Not automatically. Home visits may be appropriate for most residents receiving care in an ARC facility. However, most retirement village residents would likely not meet the criteria. For patients in Aged Residential Care, please specify the level of care.<\/p>\n<\/div><\/details><\/div>\n\n\n\n<div class=\"wp-block-atomic-blocks-ab-accordion ab-block-accordion\"><details><summary class=\"ab-accordion-title\">Do patients with disabilities automatically qualify for home visits?<\/summary><div class=\"ab-accordion-text\">\n<p class=\"wp-block-paragraph\">Not automatically. Disabilities, intellectual impairments, do not in themselves qualify a patient for a home visit. If the patient is medically unable to leave home, a request may be submitted with detailed clinical justification for review by a pathologist. We encourage you to contact the Home Visit Administration Team if you believe a specific case warrants further assessment.<\/p>\n<\/div><\/details><\/div>\n\n\n\n<div class=\"wp-block-atomic-blocks-ab-accordion ab-block-accordion\"><details><summary class=\"ab-accordion-title\">What if the patient has social anxiety or a mental health condition?<\/summary><div class=\"ab-accordion-text\">\n<p class=\"wp-block-paragraph\">A general statement of social discomfort or anxiety is not sufficient for home visit eligibility. However, if a patient has a formally diagnosed condition\u2014such as severe social anxiety disorder or agoraphobia\u2014confirmed by a psychiatrist, and this condition significantly impairs their ability to attend a collection centre, the request can be submitted with clear information detailing their condition for the pathologist\u2019s review. If the clinical details are of a sensitive nature and you do not wish to disclose this on the form, please contact the home visits team (contact details below) and they will put you through to the pathologist to seek approval.<\/p>\n<\/div><\/details><\/div>\n\n\n\n<div class=\"wp-block-atomic-blocks-ab-accordion ab-block-accordion\"><details><summary class=\"ab-accordion-title\">Do oncology patients automatically qualify for home visits?<\/summary><div class=\"ab-accordion-text\">\n<p class=\"wp-block-paragraph\">Patients currently undergoing chemotherapy or other cancer treatments do not automatically qualify for home visits. Many oncology patients are still able to attend outpatient clinics and are therefore expected to visit a collection centre unless they are medically unable to do so. However, we understand the need to protect immunocompromised individuals, and we will make every effort to: <\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Prioritise their visit on arrival<\/li>\n\n\n\n<li>Offer seating away from others when possible, and<\/li>\n\n\n\n<li>Minimise wait times. <\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">If you believe a patient\u2019s treatment has left them genuinely housebound, please provide detailed clinical justification for review.<\/p>\n<\/div><\/details><\/div>\n\n\n\n<div class=\"wp-block-atomic-blocks-ab-accordion ab-block-accordion\"><details><summary class=\"ab-accordion-title\">Why are these criteria so strict?<\/summary><div class=\"ab-accordion-text\">\n<p class=\"wp-block-paragraph\">Home visits are funded under specific criteria within the Community Pathology Services Agreement. To ensure this limited service remains available to patients with the greatest clinical need, all requests are reviewed carefully and must align with Health NZ\u2019s funding guidelines.<\/p>\n<\/div><\/details><\/div>\n\n\n\n<div class=\"wp-block-atomic-blocks-ab-accordion ab-block-accordion\"><details><summary class=\"ab-accordion-title\">Why did my patient previously qualify for home visits but now they don\u2019t?<\/summary><div class=\"ab-accordion-text\">\n<p class=\"wp-block-paragraph\">Home visit eligibility is reviewed regularly to ensure alignment with the funding criteria set by Health NZ. A patient may have previously qualified due to a temporary condition, post-operative recovery, or other clinical circumstances that have since changed. If a patient\u2019s mobility or health has improved, or if their previous condition is no longer considered medically limiting, they may no longer meet the criteria for a funded home visit.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Additionally, we acknowledge that in the past, some requests may have been approved in error due to system limitations. We have since improved our processes to ensure more consistent assessment and to better identify when patients no longer meet eligibility.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">If you believe the patient still qualifies based on updated clinical information, please submit a new request with supporting justification or contact the Home Visit Administration Team for clarification.<\/p>\n<\/div><\/details><\/div>\n\n\n\n<div class=\"wp-block-atomic-blocks-ab-accordion ab-block-accordion\"><details><summary class=\"ab-accordion-title\">Can patients or family members request a home visit directly?<\/summary><div class=\"ab-accordion-text\">\n<p class=\"wp-block-paragraph\">No. Only the referring practitioner or their authorised representative (e.g., senior nurse) may request or modify home visit bookings.<\/p>\n<\/div><\/details><\/div>\n\n\n\n<div class=\"wp-block-atomic-blocks-ab-accordion ab-block-accordion\"><details><summary class=\"ab-accordion-title\">What information must I include with a home visit request?<\/summary><div class=\"ab-accordion-text\">\n<p class=\"wp-block-paragraph\">Please include:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Detailed clinical justification for the home visit<\/li>\n\n\n\n<li>NHI number and full patient details<\/li>\n\n\n\n<li>Test(s) required and urgency<\/li>\n\n\n\n<li>Fasting\/medication instructions if relevant<\/li>\n\n\n\n<li>Referrer contact information<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Incomplete requests or those lacking justification will be processed as collection centre referrals.<\/p>\n<\/div><\/details><\/div>\n\n\n\n<div class=\"wp-block-atomic-blocks-ab-accordion ab-block-accordion\"><details><summary class=\"ab-accordion-title\">What if I believe a patient needs a home visit but doesn\u2019t meet the standard criteria?<\/summary><div class=\"ab-accordion-text\">\n<p class=\"wp-block-paragraph\">You may still submit the request with detailed clinical notes for review. These cases will be assessed by a pathologist, who will make the final decision regarding their eligibility. If you are concerned, this can be discussed with a pathologist directly ahead of submitting the form.<\/p>\n<\/div><\/details><\/div>\n\n\n\n<div class=\"wp-block-atomic-blocks-ab-accordion ab-block-accordion\"><details><summary class=\"ab-accordion-title\">How will I be notified if a home visit is declined or changed?<\/summary><div class=\"ab-accordion-text\">\n<p class=\"wp-block-paragraph\">You will receive one of the following PMS inbox notes:<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">If the request was sent via eOrder and the referral lacks sufficient clinical details:<br><em>\u201cWe have received a referral for your patient; however, it does not appear to meet the eligibility criteria for Home Visits, or the clinical details provided are insufficient. The referral has been processed as a Collection Centre request. If this information is incorrect, please contact SIHomevisits@awanuilabs.co.nz\u201d<\/em><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">If the request was sent via eOrder and the patient has been assessed as no longer eligible (e.g., found to be mobile during a visit):<br><em>\u201cWe have reason to believe that your patient may no longer meet the eligibility criteria for Home Visits and has been removed from this service.
